Pursuant to 40 CFR 403.12, Tempe City Code
§27-54, and Part 3 of your wastewater discharge
permit, all permitted Industrial Users are
required to complete an annual report and, in
addition, those industries classified as
Significant Industrial Users (SIU) are
required to complete semiannual reports pursuant
to 40 CFR 403.12(e).
The report
forms are only available in portable document
format (PDF).
However, all
PDF documents issued by Environmental can now be
saved if you are using version 8 of AdobeÓ
Reader, which can be down loaded from
http://www.adobe.com.
The semiannual report is due on July 18,
2008. An email will be issued to each SIU,
providing a link to download the semiannual form
from a secure City of Tempe site.
