This form is a method to efficiently report the routine maintenance of
the pretreatment device at your establishment. This device
may be a grease trap, interceptor or other similar device. All
users of the City of Tempe sanitary sewer collection system are
required by Division 3, Section 27-22 to provide and maintain at
the owners expense such devices.
